Preventing Medication Errors to Improve Patient Outcomes

Abstract

Medication errors are any preventable events that may cause or lead to inappropriate medication use or patient harm while the medication is in the control of the healthcare professional or patient. Although errors may occur at every stage of the medication use process, they occur more frequently during the prescribing and administration stages. It is estimated that more than 50 million medication errors occur each year in the United States across all healthcare settings and including mistakes made by patients in their own homes. Current findings support the need for health care facilities and providers to have organized, up-to-date systems for the ordering, dispensing and administering of medications. Formulating organizational policies and procedures is also imperative for minimizing and preventing medication errors, and ultimately to help ensure patient safety. This educational activity reviews the incidence of medication errors and identifies resources for obtaining best practice information. In addition, the program provides healthcare providers with insight into current strategies and guidelines to improve error detection as well as minimize their occurrence, thereby improving patient outcomes.
